¿CÓMO FUNCIONAN LOS FITOESTEROLES?

Los fitoesteroles son ingredientes alimenticios naturales presentes en plantas y se conoce desde la década de los sesenta que poseen la capacidad de reducir el colesterol LDL, según han reportado múltiples meta-análisis que el consumo diario de 2 gramos de fitoesteroles reducen el colesterol entre un 10 y 15% (Demonty, 2009; AbuMweis, 2008; Ras, 2013). Los mecanismos propuestos para la reducción del colesterol son (De Smet, 2012):

  • Compiten con el colesterol para su incorporación a la micela a nivel intestinal, lo que genera que menos colesterol entre a las células epiteliales del intestino delgado, llamados enterocitos. El colesterol que alcanza a ingresar en los enterocitos debe ser esterificado para pasar a formar las lipoproteínas que luego van a la sangre.
  • Los fitoesteroles compiten nuevamente con el colesterol para evitar este paso, inhibiendo una enzima denominada ACAT. El fitoesterol, al no entrar en la sangre, es expulsado nuevamente al lumen intestinal. En este proceso, se expulsa también una cantidad de colesterol no utilizado.
  • Los fitoesteroles además tienden a co-cristalizar con el colesterol en el intestino, evitando que se incorpore en las micelas (Rozner, 2006), y recientemente se ha descrito además un mecanismo de secreción de colesterol directamente desde la circulación al lumen intestinal (De Smet, 2012).
